Resale or exemption documentation
Complete the included certificate only when it applies. Provide any additional exemption documentation requested for your account.

Credit application
Complete this section only when requesting open-account terms. The application includes a personal guarantee. Execuprint will confirm any approved terms separately.

Card, ACH, or wire payment
For card payments, ask your Execuprint contact for a payment link. ACH or wire instructions are available upon request.

Return the non-sensitive portions to your Execuprint contact or Customer Care. Ask us how to send sensitive information securely.
Please protect sensitive information.
Do not send full card numbers, card security codes, bank account details, Social Security numbers, or similar sensitive information through standard email or the general file uploader. Contact us first so we can confirm the appropriate delivery method.
